Your first appointment will begin by advising us of your biographical, medical and dental history. These confidential forms require about 15 minutes to complete and you should arrive a little early to do so. You will need an accurate list of any medications you may be taking as well as the names of your current physicians.

We are very interested in your previous dental history and any concerns you have about that care. We encourage patients to tell us what they like, and especially dislike, about their dental experience. If X-Rays are current (5 years old or less) and available, we would like them forwarded to our office before your arrival. You will have to contact your previous dentist to arrange for your records to be released to us.
You can select any one of our three doctors to be your primary care dentist and you can choose from six dental hygienists to provide your cleanings.
In most cases, this appointment will involve a comprehensive head and neck, dental, and oral cancer exam. We will usually recommend necessary X-Rays if current films are not available (Learn more about X-Rays.) The majority of first appointments include cleaning of the teeth and a consultant to discuss any future needs. Adults should plan for about 60-75 minutes; Children about 45-60 minutes.
In most cases, this appointment will involve a problem focused exam, at least one X-Ray, and initial stabilization of the problem. All dental emergencies are different and therefore patients should plan for at least 90 minutes.
Patients under 18 years of age are considered legal minors and must be accompanied by a parent or legal guardian during their first appointment. We request that this responsible adult remain within the office area during this appointment to give consent for necessary treatment or answer medical questions. If the parent cannot remain within our office, they should notify our reception staff before the child's appointment.
If you are away from home and in need of dental care, we can help. We can work with your hometown dentist to provide emergency or routine care. We also are a primary referral provider for the Health Center at BGSU.
We often provide mid-year dental cleanings and checkups for out of town students. We can treat toothaches, infected gums, and wisdom teeth as well as traumatic injuries to the mouth. (We are the team dentists for Bowling Green State University athletics.)
If you would like us to work with your hometown dentist, we will need contact information for your dental records. Please provide us with your dentist’s full name, phone number, and e-mail address if possible.
If you plan to have help paying for your visit from parents and/or their insurance, we will need contact information for the necessary paperwork. Our office may need to contact your parent during the first visit to gain consent for your treatment and reimbursement. We encourage parents to call us anytime, if they have any questions about your care.
We welcome physically and mentally impaired patients and our facility is designed to be completely ADA compliant. We ask that caregivers accompany these patients to our office and often remain in our treatment areas. Most special needs patients are more comfortable with their caregiver present.
